FORM - Registration for submitting an application for temporary residence
A complete application with fingerprints can only be submitted during the appointment. Submitted copies of documents are certified as true copies only when submitting an application during an appointment. A stamp in the passport at the time of submitting the application can be obtained only during the scheduled visit (if the deadline for submitting the application has been met and there are no doubts as to the legality of the foreigner's stay in Poland). You should submit the application for a residence permit at the latest on the last day of your legal stay in Poland.

To meet the needs of families, we launched application form thanks to which you can register your family to submit applications for a temporary residence permit, a permanent residence permit and a long-term EU resident permit at one time. An application for a residence permit may be submitted via the post office or the filing office at the seat of the Faculty or at a branch office. The date of submission of the application shall be the date of submission of the application to the Office or the date of the postmark. When submitting an application in this way fingerprints are not collected, and submitted documents are not verified for compliance with the original. It is also not possible to put a stamp in the passport thenconfirming the submission of the application. If you send the application by post or submit it to the registry office, you will have to wait for the Office's call, and the process of legalizing your stay will be much longer. Then, after receiving the summons, you will be obliged to appear in person on the indicated date, in order to, among other things, fingerprinting.
The procedure will be initiated only after the formal deficiencies in the application have been supplemented, in accordance with the request sent by the Office. The waiting time for the first call depends on the number of applications awaiting registration and can range from a few to several weeks. This also extends the waiting time for a decision to be made.

By booking a date for a visit to the Mazowieckie Voivodship Office in the Department of Foreigners' Affairs in matters of submitting an application for legalization of stay, the Customer declares that by providing his name, date of birth, telephone number, citizenship, he consents to the processing of his personal data by the Mazowieckie Voivode in order to carry out the process online booking of appointments pursuant to art. 6 sec. 1 point a) of Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of individuals with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46 / EC (Journal EU Office L 119 of May 4.05.2016, XNUMX).
During the visit booking process, the following data should be provided: name, surname, date of birth, citizenship, number of the currently held residence permit, its expiry date, name and surname of the representative in the case, contact telephone number.
- The above-mentioned data are obligatory for verification on the day of submitting documents to the Office and obtaining a ticket number.
- Applications for a temporary residence permit are accepted in Warsaw at ul. Marszałkowska 3/5, Plac Bankowy 3/5 (entrance G) and Al. Jerozolimskie 28
- Applications for a permanent residence permit and a long-term EU resident permit are accepted only at the Office at ul. Krucza 5/11.
- A person registered in the online booking system is obliged to appear at the Office in front of the indicated position, at least 10 minutes. before the planned visit.
- More than 5 minutes late may result in refusal to accept documents at the Office.
- The registered person must appear in person for the appointment. The foreigner's lack of personal appearance results in the refusal to accept the application. The only exception is the appearance of minors under 6 years of age.

- After making an online reservation, it is not possible to change the data of the saved person.
- In the case of making more than one reservation for the same personal data - the administrator reserves the right to delete the second and subsequent entries.
- Providing incomplete data in the form, untrue data, or entering the wrong queue or location will result in the cancellation of the reservation or absolute refusal to accept the application.
- If an attorney is involved in the case, his or her presence is required when submitting the application. The absence of the representative results in the refusal to accept the power of attorney in the case.

- Indicating an address of residence other than the territory of the Mazowieckie Voivodeship results in the refusal to accept the application at the Mazowieckie Voivodeship Office.
- Entering offensive, ridiculing or violating the rules of social coexistence into the booking form will result in the cancellation of the booking and blocking of the user's account.

- The form is used to make joint reservations for families: married couples and children or foreigners with family members who join it and submit applications at the same time. It is not possible to register individual people - in such a case, registration must be made via this form.
- The order of registration is determined by the date of legal stay, which is verified by employees when submitting the application. The date of the visit is determined by the Office and cannot be changed. It is possible to cancel the visit (point 8).
- Notification about booking an appointment is sent only via e-mail.

SUBMISSION OF APPLICATIONS OTHER THAN DURING AN ARRANGEMENT OF VISIT (BY REGISTRATION OFFICE OR BY POST)
your request IS NOT COMPLETE, is burdened with formal defects that make it impossible to start the proceedings (e.g. presenting the original passport).
Be prepared for a longer waiting period for registration and a call to complete formal deficiencies (about 2-3 months, waiting time for the first call may depend on the number of submitted applications)
You will find out about the case number via SMS, and you will receive the login and password for electronic checking of the case status with the first summons.
Formal deficiencies that you will be requested to complete must be submitted within 7 days from the date of receipt of the summons, otherwise your application will not be considered.
If you submit formal deficiencies, they are analyzed in terms of the correctness of the submitted documents as well as the date of their submission, which involves waiting for a return confirmation of receipt or initiating a complaint procedure - this stage takes another 2 months.
Only after formal verification, the case can be examined on its merits.
Submission of a set of documents allows for faster processing of the application.
PASSPORT STAMPS
According to Art. 108 of the Act on Foreigners, a stamp in the passport is placed when two conditions are met jointly: the application was submitted within the time limit provided for in the Act, i.e. by the last day of legal stay and does not contain any formal defects or the defects were supplemented on time.
The stamp can be obtained at the time of submitting the application in person, regardless of whether the foreigner has a valid residence card or visa, provided that it can be unquestionably established that the application was submitted during the foreigner's legal stay (this involves, among others, correct reading of the stamps crossing the border, determining the authenticity of the visa, whether the foreigner may stay in Poland on the basis of a document issued by another Schengen state, etc.). They stamped in the passport, despite submitting the application in person, PERSONS WHO APPLY DURING ILLEGAL STAY WILL NOT BE RECEIVED.
Persons who have submitted an application by post, at a registry office or at the registration office may obtain a stamp in the passport only after completing the formal verification of the application (CORRECTNESS OF THE SUBMITTED DOCUMENTS AND THE DEADLINE FOR THEIR SUBMISSION), when the case is examined in terms of its merits, provided that unquestionably determine that the application was submitted during the foreigner's legal stay (this involves, among others, the correct reading of border crossing stamps, determining the authenticity of the visa, determining whether the foreigner may stay in Poland on the basis of a document issued by another Schengen state, etc.).
Note: THE STAMPILA CONFIRMS THE APPLICATION FOR TEMPORARY, PERMANENT RESIDENCE OR LONG-TERM RESIDENCEwhile DOES NOT ENTITLE you to travel within the Schengen area. On the basis of stampils, it is possible to leave the territory of the Republic of Poland to a third country, return to Poland in such a situation will be possible only after obtaining the appropriate visa or as part of visa-free travel, if the foreigner's country of citizenship has been covered by visa-free travel.
