When to apply?
Proposal you apply for a residence registration certificate if you are a citizen of the UK/beneficiary of the Withdrawal Agreement, you do not have a document confirming the registration of your stay issued after December 31.12.2020, 31.12.2020, and you stayed in Poland before December 3, XNUMX and continue your stay, which lasts over XNUMX months.
You submit the application personally. The requirement to submit an application in person does not apply a minor.
Proposal on behalf of a minor EU citizen are submitted by parents or court-appointed guardians or one of the parents or one of the court-appointed guardians.
If you submit proposal without formal deficiencies or you complete the formal deficiencies within the time limit indicated by us, the proceedings will be initiated.
You are entitled to stay for a period longer than 3 months if you meet the following conditions:
- you are an employee or entrepreneur;
- you are covered general health insurance or you are a person entitled to health care services on the basis of the provisions on coordination within the meaning of Art. 5 point 23 of the Act of 27 August 2004 on health care services financed from public funds (Journal of Laws No. 210, item 2135, as amended), or you have private health insurance covering all expenses that may result during a stay in the territory of the Republic of Poland in connection with the need for medical assistance or hospital treatment, in which the insurer undertakes to cover the costs of health services provided to the insured person directly for the benefit of the entity providing such services, on the basis of a bill issued by this entity and you have sufficient financial resources to maintain themselves and family members on the territory of the Republic of Poland, so as not to constitute a burden for social assistance;
- you are studying or undergoing vocational training in the Republic of Poland and you are covered general health insurance or you are a person entitled to health care services on the basis of the provisions on coordination within the meaning of Art. 5 item 23 of the Act of 27 August 2004 on health care services financed from public funds, or you have private health insurance that covers all expenses that may arise during your stay in the territory of the Republic of Poland in connection with the need for medical assistance or hospital treatment, in which the insurer undertakes to cover the costs of health care services provided to the insured person directly to the entity providing such services, on the basis of an invoice issued by that entity, and you have sufficient financial resources to support yourself and your family members in the territory of the Republic of Poland, so as not to constitute a burden on social assistance;
- you are the spouse of a Polish citizen;
and
- you used right of residence on the territory of Poland during the transition period and you met the conditions of stay;
- your stay on the territory of Poland on the day of submitting the application is uninterrupted;
Where to submit the application?
Correctly and completely filled out proposal deposit in Polish personally at the Department for Foreigners in Warsaw at ul. Marszałkowska 3/5, mezzanine, stand 21 (take a number with the letter "I").
You also have the option of submitting a fully and correctly completed application in paper form - by post. You can also submit the application through the registry office, ul. Marszałkowska 3/5 Warsaw, pl. Bankowy 3/5 Warsaw.
If you submit your application by post or through the application office, you will have to wait for the Office's summons. Then, after receiving the summons, you will be obliged to appear in person on the date indicated in the summons. Original documents must be presented during the visit. You can also submit other required documents.
What documents to submit?
Documents necessary to initiate the procedure (so-called formal requirements):
- proposalin three copies, completed in Polish, in accordance with the instruction;
- 4 Photos;
- photocopy a valid travel document or another document confirming identity and citizenship (original available for inspection);
- fingerprinting (in the case of minors, fingerprints are taken from children who have reached the age of 6 on the day of submitting the application).
Documents needed to register the stay of a UK citizen:
- confirming use residence rights during the transitional period on the territory of Poland;
- confirming uninterrupted stay on the territory of Poland on the day of submitting the application;

When working, you must submit:
- a written statement of the employer or the entity authorized to entrust the EU citizen with the performance of work about the intention to entrust him with the performance of work or a certificate of performance of work;
- in the case of self-employment - a written statement regarding the entry in the National Court Register or the Central Register and Information on Economic Activity or other proof that you are a self-employed person in the territory of the Republic of Poland;
In the case of studying or undergoing vocational training, the following should be submitted:
- a certificate from a state or non-state higher education institution on admission to studies or referral to vocational training;
- supporting document having health insurance or the right to use health care services on the basis of the provisions on the coordination of social security systems or a document confirming possession of private health insurance covering all expenses that may arise during the stay in the territory of the Republic of Poland in connection with the need for medical assistance or hospital treatment, in which the insurer undertakes to cover the costs of health services provided to the insured, directly to the entity providing such benefits, on the basis of a bill issued by that entity;
- a written statement that you have sufficient financial resources to support yourself and your family members so as not to be a burden on social assistance, or proof of having them.
- Evidence confirming their possession may be in particular:

certificate of the limit on the credit card issued by the bank that issued the credit card - issued not earlier than one month before the date of submitting the application for registration of stay;

a certificate of having cash in a bank or cooperative savings and credit union, based in the territory of the Republic of Poland or another Member State of the European Union, a Member State of the European Free Trade Association (EFTA) - a party to the Agreement on the European Economic Area, the Swiss Confederation or the United Kingdom Great Britain and Northern Ireland;

a document confirming the award of a domestic or foreign scholarship;

certificate of employment and earnings.

In cases other than those indicated above, the following should be submitted:
- supporting document having health insurance or the right to use health care services on the basis of the provisions on the coordination of social security systems or a document confirming possession of private health insurance covering all expenses that may arise during the stay in the territory of the Republic of Poland in connection with the need for medical assistance or hospital treatment, in which the insurer undertakes to cover the costs of health services provided to the insured, directly to the entity providing such benefits, on the basis of a bill issued by that entity;
- proof of having sufficient financial resources to support yourself and your family members, so as not to constitute a burden for social assistance. Evidence confirming their possession may be in particular:

In the event that a family member of a UK citizen who is a UK citizen who used the right of residence in the territory of Poland during the transition period and continues his/her stay - the following should be submitted:
- certificate of registration of residence of a ZK citizen, issued to a ZK citizen with whom a family member resides on the territory of the Republic of Poland;
- spouse – a document confirming the marriage (e.g. marriage certificate),
- descendant – a document confirming the existence of family ties and a document confirming age or being dependent on a ZK citizen or his/her spouse;
- preliminary a citizen of the United Kingdom or his spouse - a document confirming ties and a document confirming being dependent on a citizen of the United Kingdom or his spouse;
- parents a minor citizen of the United Kingdom - a document confirming that the minor citizen of the United Kingdom is actually cared for and a document confirming that the minor citizen of the United Kingdom is dependent on him/her.
In the case of marriage with a Polish citizen, the following should be submitted:

a document confirming marriage to a Polish citizen;

a photocopy of a document confirming the citizenship and identity of a Polish citizen - spouse (identity card or passport) - the original should be presented for inspection or its notarized certification should be attached.
How to pick up the document?
When collecting the document, the presence of a child who is 6 years old by the date of submitting the application for the document is required.
Customer reception hours: Monday 10:00-17:30, Tuesday - Friday 8:00-15:00.
If the certificate is lost, a new document is issued.
What should I know?
Before the deadline for filing an appeal expires, the party may waive the right to appeal against the public administration body that issued the decision.
On the day of delivery to the public administration body of the statement on the waiver of the right to appeal by the last party to the proceedings, the decision becomes final and binding.
If the right to appeal against the decision is waived, there is no right to appeal to the administrative court.

You should report to the voivode competent for the place of your residence in the case of:
- changes to the data contained in the document,
- document loss,
- damage or other circumstances that make it difficult to determine the identity of the holder.
