When to apply?
Proposal you apply for a document confirming the right of permanent residence of an EU citizen personallyafter 5 years uninterrupted stay on the territory of the Republic of Poland. EU citizen being spouse of a citizen of the Republic of Poland acquires the right of permanent residence after 3 years of continuous residence on the territory of the Republic of Poland.
The requirement to submit an application in person does not apply a minor.
In a particularly justified, motivated case, including due to your health, you may waive the obligation to appear in person.

Where to submit the application?
Correctly and completely filled out proposal deposit in Polish personally at the Department for Foreigners in Warsaw at ul. Marszałkowska 3/5, mezzanine, stand 21 (take a number with the letter "I").
If you are unable to submit the application in person, please send a complete and correctly completed application in paper form by post. You can also submit via the application office, ul. Marszałkowska 3/5 Warszawa, documents submitted to the filing office are not verified and are not confirmed to be true copies of the originals. The service is available after prior reservation of the date and time e-service portal. By booking an appointment, you accept at the same time registration regulations.
If you submit your application by post or through the application office, you will have to wait for the Office's summons. Then, after receiving the summons, you will be required to appear in person on the date indicated in the summons in order to present the original documents. What documents to submit?
Documents necessary to initiate the procedure (so-called formal requirements):
- proposal in one copy, completed in accordance with the instruction;
- 4 Photos;
- photocopy a valid travel document or another document confirming identity and citizenship (original available for inspection).
Documents needed to confirm the right of permanent residence of an EU citizen:
- documents confirming 5 years uninterrupted period of residence on the territory of Poland (3 years in the case of a spouse of a citizen of the Republic of Poland) together with documents confirming the fulfillment of the conditions of stay during this period;
- documents stating the reasons for breaks in stay, if any, together with documents confirming that the EU citizen met the conditions of stay during this period.
How can I get a document?
You can pick up the document in person at the seat of our Department, stand 21, ul. Marszałkowska 3/5 Warszawa, you need to take a ticket with the letter "V".
The document may be collected by the proxy, on the basis of a special power of attorney to collect it, upon presentation of a document confirming his/her identity.
Customer reception hours: Monday 10:00-17:30, Tuesday - Friday 8:00-15:00.
In case of loss of the document confirming the right of permanent residence, a new document is issued.
What should I know?
The party may appeal against the negative decision to the Head of the Office for Foreigners - through the Mazowieckie Voivode - within 14 days from the date of its delivery.
During the period for lodging an appeal, the party may waive the right to appeal against the public administration authority that issued the decision.
On the day of delivery to the public administration body of the statement on the waiver of the right to appeal by the last party to the proceedings, the decision becomes final and binding.
If the right to appeal against the decision is waived, there is no right to appeal to the administrative court.

What are my obligations after issuing the document?
You should report to the voivode competent for the place of your residence in the case of:
- changes to the data contained in the document,
- damage or other circumstances that make it difficult to determine the identity of the holder,
- expiry date of the document.
