When should I apply?
residence card you exchange if:
- the data included in the current residence card will change, e.g. name, registered address;
- your appearance will change to such an extent that the photograph included in the current residence card will not allow for your identification;
- you have lost your current residence card;
- the current residence card has been damaged;
You submit the application for replacement of the residence card in person, within 14 days from the occurrence of the above-mentioned circumstances. The application on behalf of a minor foreigner is submitted by parents or court-appointed guardians or one of the parents or one of the court-appointed guardians. When submitting an application, a minor who is over 6 years of age must be present.

Where to submit the application?
A correctly and completely completed application should be submitted in person to the Department of Foreigners' Affairs in Warsaw, ul. Marszałkowska 3/5 (mezzanine, stand 17, room A09), after completing the application and booking a visit via foreigner's portal InPOL.
Reception hours: 10:00-17:30 (Monday), 8:00-15:00 (Tuesday - Friday).
The "E" ticket applies only to submitting an application for another permanent or long-term resident's EU residence card, issuing a residence card for a foreigner's family member who has been granted a residence permit on the basis of article 159 of the Act on foreigners, replacement of a stolen or damaged card or in the event of a change in the data provided on it. Customers scheduled for an appointment in the wrong queue will not be served.

- WSC customer service is provided solely on the basis of a number collected from a ticket machine or an online visit booking.
- there is a daily limit - 100 numbers issued by the ticket machine for number C - correspondence submitted in person to the Department of Foreigners' Affairs. You can submit up to 8 cases per number.
- Online reservations for visits are available on the website www.e-uslugi.mazowieckie.pl. Since September this year, online visits for number C can be scheduled every 15 minutes. Up to 8 cases can be submitted during one visit.

What documents do I need to attach to the application?
Documents needed to initiate the procedure (so-called formal requirements):
- application in one copy, completed in accordance with the instruction via foreigner's portal InPOLin the case of a minor child, signed by one of the parents or a court-appointed guardian;
- 2 Photos,
- a photocopy of a valid travel document (original available for inspection), in particularly justified cases, when you do not have a valid travel document and you are unable to obtain it, you can present another document confirming your identity;
- to start the procedure, you will also need confirmation that you have paid for the replacement of the document and submission fingerprints.
Remember that photocopies of all pages containing visas, annotations, entries, etc. must be submitted with the presented original of the travel document. inability to obtain a travel document issued by the competent authorities of the country of origin should be confirmed by a certificate from the embassy.
Documents needed to replace the residence card:
- a current copy of the civil status act in the event of a change in the data included in the current residence card (e.g. change of surname);
- confirmation of registration for permanent or temporary stay lasting more than 2 months;
- notification of the loss of the residence card issued by the Police unit (optional) or the Foreigner's declaration regarding the circumstances of the loss/damage of the residence card submitted under pain of criminal liability (form);
- a photocopy of the decision on granting a residence permit in Poland;
- a photocopy of your last residence card.
If you send a notification about the loss/damage of a document additionally on a special form – you will receive a free certificate confirming the fact of reporting the loss/damage of the document. This certificate is valid until the next document is issued, but not longer than for a period of 2 months from the date of issue.
All documents must be submitted in original or certified copy. Use the cover letter when sending documents to the Department of Foreign Affairs.
Residence card fees
The reduced fee is PLN 50
You can take advantage of the relief if you are:
- a student
- high school student
- a minor child under the age of 16
- in a difficult financial situation
The fee for replacing the residence card in the event of culpable loss or culpable destruction is PLN 200,00
The fee for replacing the residence card in the event of another culpable loss or another culpable destruction is PLN 300,00

How long will I wait?
The residence card shall be replaced within 30 days of receipt of the complete set of documents. In a situation where the current residence card was issued by another voivode, the card replacement procedure may take 2-3 months. If it is necessary to conduct additional explanatory proceedings, you will be notified in writing of the expected date of issuing the document and the reasons for the delays.

What should I know?
Attention! Completing the application electronically is not tantamount to submitting it online.
They are downloaded when submitting the application fingerprints in order to encode them in the residence card;
Failure to submit fingerprints will result refusal to initiate proceedings on issuing a residence card;
If it is physically impossible to collect fingerprints from a foreigner, the administration authority refrains from collecting them;
You should submit the documents in Polish in the originals or in photocopies together with the originals for inspection by an employee of the department/office;
Documents in a foreign language should be accompanied by translations into Polish, made by a Polish sworn translator;
The residence card confirms the foreigner's identity and entitles him, together with the travel document, to cross the border multiple times;
The data on the residence card is entered in accordance with the entries in the Foreigner's valid passport.
You collect your residence card in person at the Department of Foreign Affairs in Warsaw, ul. Marszałkowska 3/5, stand 12. When collecting a residence card issued to a minor who has turned 13, their presence is required. If the residence card was issued to a minor who has not turned 13 by the date of its collection, the card is collected by their legal representative or curator.
To collect your residence card, take with you a valid travel document and a previous residence card, if it was issued.
