Replacement of the residence card of a family member of an EU citizen and a citizen of the Republic of Poland

When to apply?

Make proposal for issuing a new residence card when the current document expires. The application should be submitted at least 30 days before the expiry of the current document.

Make proposal for the replacement of a residence card, if the document you currently have has been damaged, you have lost it or the data contained in it has changed. The application must be submitted within 14 days after the occurrence of the above-mentioned conditions for their replacement.

You submit the application personally. When submitting an application a minorwho is over 6 years of age, is required to be present. The application on behalf of a minor EU citizen is submitted by parents or court-appointed guardians or one of the parents or one of the court-appointed guardians.

If you apply without formal deficiencies or you complete the formal deficiencies within the time limit indicated by us, the proceedings will be initiated.

Where to submit the application?

Correctly and completely filled out proposal deposit in Polish personally at the Department for Foreigners in Warsaw at ul. Marszałkowska 3/5, mezzanine, stand 21 (take a number with the letter "I").

You also have the option to submit a complete and correctly completed submission proposal in paper version - by post. You can also submit an application via the application office, ul. Marszałkowska 3/5 Warsaw. Documents submitted to the Registration Office are not verified and are not confirmed to be true copies of the originals. The service is available after prior reservation of the date and time e-service portal. By booking an appointment, you accept at the same time registration regulations.

If you submit proposal by post or through the registry office, you will have to wait for the Office's call. Then when you receive the summons, you will be required to appear personally within the time specified in the call. Original documents must be presented during the visit. You can also submit other required documents

What documents to submit?

Documents needed to exchange / issue a new residence card:

  • proposal in one copy, completed in Polish, in accordance with the instruction, in the case of a minor signed by one of the parents or a court-appointed guardian;
  • 2 Photos;
  • photocopy a valid travel document (original available for inspection);
  • submission fingerprints (in case of minors fingerprints are taken from children who are 6 years old on the day of submitting the application);
  • residence card of a family member of an EU citizen / residence card.
All documents are submitted in original/certified copy. You can complete the missing documents in person at our Department, ul. Marszałkowska 3/5 Warsaw, stand 21, ticket with the letter "I" or through the registry office of the office, ul. Marszałkowska 3/5 Warsaw, pl. Bankowy 3/5 Warsaw.

How can I get a residence card?

You pick up your residence card personally at the headquarters of our Department, stand 21, ul. Marszałkowska 3/5 Warszawa, you need to take a ticket with the letter "V".

If a residence card has been issued to a child who is under 13 years of age by the date of its collection, the document is collected by a parent, legal guardian or custodian, respectively, upon presentation of a valid document confirming his/her identity.

When collecting the residence card, the presence of a child who is 6 years old by the date of submitting the application for the document is required.

Customer reception hours: Monday 10:00-17:30, Tuesday - Friday 8:00-15:00.

In case of loss of the residence card, a new document is issued.

What should I know?

The party may appeal against the negative decision to the Head of the Office for Foreigners - through the Mazowieckie Voivode - within 14 days from the date of its delivery.

During the period for lodging an appeal, the party may waive the right to appeal against the public administration authority that issued the decision.

On the day of delivery to the public administration body of the statement on the waiver of the right to appeal by the last party to the proceedings, the decision becomes final and binding.

If the right to appeal against the decision is waived, there is no right to appeal to the administrative court.
