What is worth remembering when submitting an application?

Please read the instruction before completing the application. Fill out the application meticulously and slowly - the questions in the application are translated into three languages, in case of doubt, seek the help of an interpreter.

When completing the permit application, remember to:

  • fill it out legibly in Polish. If you do not speak Polish, use the help of an interpreter
  • complete all required fields of the application in accordance with the facts. Do not omit anything, withholding information may adversely affect the processing of your application
  • fill it with capital letters in the appropriate boxes
  • inform us if you have provided other personal data in the past - write about it in the justification. If you have changed your name, please provide your previous personal details as well
  • indicate the place of actual residence, where the correspondence will be received. If your mailing address is different, please let us know when submitting your application
  • in the criminal record section, indicate information on the sentences issued against you, and in the event of uncertainty as to the sentences issued, indicate that the proceedings are pending (NOTE: payment of the fine does not mean that no criminal proceedings have been initiated and no sentence has been issued in the case )
  • in the part regarding pending criminal proceedings or proceedings in petty offense cases, provide information on all pending proceedings (e.g. in the case of unaccepted fines)
  • do not hide information, the application is submitted under criminal liability - so if you have been convicted, check the appropriate box
  • put a handwritten signature under it and enter the name and surname using the Latin alphabet
  • provide contact details, i.e. telephone number and/or e-mail address, which will make it easier for us to contact you
