Declaration form on entrusting work for persons who received a residence permit under the Act of 17 December 2021 amending the Act on foreigners and certain other acts (Journal of Laws of 2022, item 91) - information on the need to submit statements can be found in the decision instruction. The declaration is completed by the employer. The deadline for submitting the statement is 60 days from the date of receipt of the decision.
