Medical insurance

Insurance that covers the costs of treatment in Poland; thanks to it you can go to the family doctor, to the hospital; this is not accident insurance. It is about insurance that will enable you to use health and hospital care (you can use both doctor's care at the clinic and hospital treatment), or a current insurance policy, indicating that the insurer will cover the costs of treatment. The document confirming the insurance may be:

  • insurance contract with the National Health Fund
    Submit the original insurance contract with the National Health Fund for inspection, together with proof of payment of insurance premiums (from the date of conclusion of the contract to the date of issuing the decision).
    Remember that you have to pay each premium by the 15th day of the following month!
  • the right to use health care services based on the provisions on the coordination of social security systems, e.g. in the Social Insurance Institution - ZUS
    Submit the confirmation of the application to the Social Insurance Institution together with the confirmation of payment of contributions to the Social Insurance Institution.
  • foreign insurance
    If the insurance comes from a country other than Poland, it must be translated into Polish by a sworn translator and presented together: the original of the insurance together with the original of the translation
  • private insurance policy

The policy must cover the costs of treatment in the territory of the Republic of Poland (i.e. that you can use the help of a doctor both in the clinic and in the hospital). Accident insurance is not proof that you have insurance covering medical expenses. Remember, it must be valid on the date of the decision.
In the case of installment payments for the policy, attach proof of payment (original available for inspection).

Health insurance (visa)

health insurance or travel medical insurance with a minimum insurance amount of EUR 30, required in the visa extension procedure. The insurance must be valid for the entire period of the planned stay in the territory of the Republic of Poland and cover all expenses that may arise during your stay in Poland, as well as expenses related to the need to travel back for medical reasons, the need for urgent medical assistance, emergency hospital treatment or death .

Remission

Completion of processing your application without determining whether you meet the requirements for granting the permit you applied for, e.g. if you resigned from further stay in Poland and want to complete the procedure of legalizing your stay. From decision regarding the discontinuation of the proceedings, you can cancel.

Civil law contract

Declaration of will of two parties, e.g. persons to perform specific activities, types of civil law contracts and the rules for their conclusion are regulated by the provisions of the Civil Code, civil law contracts include, among others: mandate contract, contract work.

Employment contract, order, work

The contract under which you work/provide a service/perform a work.

Internship agreement

The agreement on the basis of which the foreigner will carry out the internship should be concluded in writing with the internship organizer and should specify:

a) a description of the internship program, including information on its educational purpose or educational components, theoretical and practical training, the position where the internship will be held, the language in which the internship will be held, the level of language proficiency necessary to complete the internship, the scope and type of tasks performed, scope of knowledge, practical skills and professional experience to be acquired,

b) duration of the internship,

c) the conditions for carrying out and supervising the internship, including the determination of the place of the internship and the appointment of the trainee's supervisor,

d) internship hours,

e) the rights and obligations of the parties regarding:

  • covering the costs of the internship,
  • necessary medical examinations,
  • accident insurance,
  • holidays,
  • conditions for termination of the contract,

f) the method of confirming the acquired knowledge, practical skills and professional experience.

Volunteer agreement

Agreement under which you will perform services as a volunteer. The contract should specify:

  1. description of volunteering,
  2. duration of volunteering,
  3. conditions for carrying out and supervising volunteering,
  4. volunteer hours,
  5. funds to cover the costs of your living and accommodation and the minimum amount of pocket money you will receive,
  6. the training you need to go through to volunteer.

Authorization to receive information about the status of the case

A written statement authorizing a designated person to obtain information on the status and progress of your case. The scope of the authorization covers only access to information. Therefore, it should not be equated with power of attorney. 

Commune/city office

Local office in the commune/city where you live. You can get it at the office confirmation of registrationand other official matters.

Registry Office

It is an office where activities in the field of civil status registration (birth, marriage, death) are carried out. At the Registry Office, you can obtain complete copies and abbreviated copies of civil status records, certificates of civil status, and you can supplement civil status records if they do not contain all the required data. As a result of the transcription (location) of a foreign civil status record, a Polish civil status record can be obtained from this office. The head of the Registry Office may also accept declarations necessary to recognize paternity or declarations of spouses on the surname of their common child or declarations of a divorced spouse on returning to the surname he had before marriage. You can also change your name or surname at the selected Registry Office if you are a Polish citizen.

Official certificate of knowledge of the Polish language

It refers to one of the conditions that must be met in order to obtain a long-term resident's EU residence permit or to acquire Polish citizenship by being recognized as a Polish citizen. The official confirmation of the knowledge of Polish as a foreign language is the certificate of knowledge of the Polish language issued by the State Commission for the Certification of Proficiency in Polish as a Foreign Language at the language proficiency level of at least B1. However, foreigners who graduated from a school in Poland and hold a certificate of completion or graduated from a school outside the Republic of Poland with Polish as the language of instruction are exempt from the obligation to present a certificate of Polish language proficiency. Therefore, the official confirmation of knowledge of the Polish language is also a certificate of completion of a school in Poland (e.g. a primary school, junior high school, high school, technical secondary school or a bachelor's or master's diploma with a supplement, which shows that the language of instruction was Polish) or a school leaving certificate for abroad with Polish as the language of instruction (e.g. Secondary School No. 8 with Polish as the language of instruction in Wołkowysk or Secondary School No. 24 named after M. Konopnicka in Lviv and others).

Establishing paternity

It takes place by acknowledging paternity or by virtue of a court decision. Acknowledgment of paternity takes place when the man from whom the child originates declares before an authorized body that he is the child's father. However, a judicial determination of paternity may be requested by the child, his mother or the alleged father of the child. In the acquisition of Polish citizenship by a child by operation of law, only those changes in the determination of paternity that occurred within one year from the date of birth of the child are taken into account.

Regulations that define the conditions of entry and stay of foreigners on the territory of Poland and the criteria for individual grants residence titles.

Certified copy of the document (granting Polish citizenship)

It is a copy of the document certified as a true copy by an authorized person, i.e. a Polish notary or Polish consul. The certification includes the notary's or consul's signature, date and place of certification. If the document contains special features (additions, corrections or damages), this is usually stated in the certification.
