A copy of a birth certificate, marriage certificate or death certificate issued by a Polish Civil Registry Office based on Polish civil status records. Entries in the civil status register about a birth, marriage or death may contain other later entries that affect the content or validity of such a certificate, e.g. in the case of a birth certificate, it may be a later formal change of personal data, and in the case of a marriage certificate, a divorce or death of a spouse. Foreigners who have foreign civil status certificates and want to apply for Polish citizenship must transfer their foreign civil status certificates to the Polish civil status register by transcription (transcription of a certificate is also its location).
