EU citizens

A document confirming the registration of an EU citizen's stay in Poland, valid indefinitely.
The stay is uninterrupted if all stays outside Poland did not exceed 6 months in total. This means that your trips outside Poland at the indicated time should not be longer than those described above. However, there are exceptions to this rule. If you are on duty or your trip is related to an exceptional personal situation, e.g. pregnancy, childbirth, illness or other important circumstances related to a business trip, studies or vocational training, which in total did not exceed 12 months, write about it in your application.

A document confirming the right to use health care in all Member States of the European Union and the European Free Trade Association.
A document showing that you stayed in Poland; these can be e.g. PIT, employment certificates, employment contracts, etc.
A spouse of an EU citizen, a child of an EU citizen or his/her spouse up to 21 years of age or dependent on an EU citizen or his/her spouse, a parent of an EU citizen or his/her spouse who is dependent on an EU citizen or his/her spouse. A family member is both an EU citizen and a foreigner without the citizenship of an EU country.
