If you are entrusted with the performance of work by an organizational unit without legal personality or a company, Annex No. 1 should be signed by a person authorized to represent this unit or company or a proxy designated by this person. In such a case, attach an appropriate power of attorney to the application. In the case of companies, Annex No. 1 should be signed by a person designated to represent the company indicated in the National Court Register (KRS).
