- submitting applications on the wrong forms (e.g. on the application form for a residence card) or without an application form (e.g. only Appendix 1);
- failure to complete all required fields in the application;
- failure to complete/or incorrect selection of criminal record information in the application (page 6 point VIII of the application);
- no legible signature of the person authorized to represent the entity entrusting the work under Annex 1;
- no indication in Annex No. 1 of the number of working hours per week in the case of contracts for services; if the hourly rate is indicated without specifying the number of working hours of the foreigner, we have no way of determining whether you have sufficient income/means to live on;
- the employer's failure to complete the information on criminal record contained on page 4 in Annex 1, i.e. points 1-7;
- submitting copies of unconfirmed documents for compliance with the original;
- submitting information to the starosta in which the remuneration specified by the employer is set by the PUP at a level below the average or the employer's lack of consent to direct persons registered with the PUP - which are grounds for refusing to grant a temporary residence and work permit.
