From August 11, 2023, a new model application for the issue or replacement of a Polish travel document for a foreigner has been in force. The application form can be downloaded below.

We kindly inform you that the failure of the appointment booking calendar has been removed. 

New dates are added to the calendar every day during office hours.

The legal stay of Ukrainian citizens (and certain members of their families) who came to Poland in the period from February 24, 2022 in connection with military operations conducted in Ukraine will be extended until March 4, 2024, and in some cases until August 31 or On September 30, 2024, the act amending the current regulations was published in the Journal of Laws and will enter into force on June 27 this year.

Last year, the Office for Foreigners started a series of meetings aimed at providing information on the legalization of stay. The next information Saturday will take place on June 17 at 9 a.m. to 15 p.m. at the seat of the office in Warsaw at ul. Taborowa 33.
